About the role:

  • Support clients to stay living in their home and connected to their community
  • Enable clients’ social connectedness, and provide services such as domestic assistance, personal care and respite care, supporting client independence at home
  • Embody a person-centred approach, recognizing and promoting the individual’s unique goals and preferences
  • Play an important role in the team by contributing to the assessment and care planning of each client as required
  • Effectively communicate with your team, to ensure the best outcomes for each client

It is a mandatory requirement for a team member working in the Aged Care sector to have a valid Police Check or a valid National Disability Insurance Scheme (NDIS) check, or potentially both, to undertake the inherent requirements of the role.
