- The Strategic Solutions & Partnerships (SSP) department is seeking an experienced and motivated individual to join our team as a Supervisor, Hospital Partnerships. In this role, you will play a vital part in overseeing program operations, providing education and clinical supervision, and ensuring the delivery of exceptional client care. Your responsibilities will include offering on-call support, occasional direct clinical care, and in-home supervision, while also managing projects and driving the successful implementation of innovative care programs.
- A key focus of this role is monitoring practice standards, client care outcomes, and resource utilization, particularly in the delivery of Personal Support Worker (PSW), Rehabilitation (OT, PT, DT, SW, SLP), and Nursing services within designated programs. You will lead initiatives within Strategic
- Solutions & Partnerships (SSP), collaborating with hospitals, home care providers, and other health sector partners to implement and evolve integrated and bundled care programs that deliver innovative solutions to meet client needs.
- With a strong commitment to client- and family-centered care, you will work closely with program funders and interdisciplinary teams, both internally and externally, to provide clinical leadership and expertise. Your efforts will aim to enhance care quality, expand service capacity, and improve overall quality of life for clients, while contributing to the development and continuous improvement of programs that meet the highest standards of care.

Key Skills
- Registered Healthcare Professional (in good standing with a professional college)
- Preference will be given to candidates with a Graduate Degree, and/or educational background in
- Project Management and/or Health Administration
- Ongoing membership with professional associations an asset
- 5-years progressive experience in respective discipline of which 2 years include experience in a supervisory role
- Good understanding of the health care services and delivery processes in the community/home care sector
- Strong clinical practitioner with an ability to use knowledge-transfer techniques to put evidence-based knowledge into practice
- Excellent leadership and facilitation skills and the ability to coach, and motivate inter-disciplinary teams
- Ability to model and lead a team in the provision of client and family-centered care, including client/ family-centered assessment, documentation, and advocacy
- History of project management or program leadership
- Demonstrated knowledge and/or skills in the use of information technology for electronic clinical documentation and clinical decision support
- Awareness and understanding of all relevant legislation related to practice standards, standards of safety and quality, and employment standards including human rights legislation
- Demonstrated leadership skills with strong organizational and self-management skills, and with demonstrated ability to achieve personal, professional and organizational performance targets
- Superior interpersonal, verbal and written communication skills, and excellent skills in customer service
- Clear, logical thinker and innovative problem solver with demonstrated advanced judgement and decision-making skills
- Demonstrated ability to work independently
- Demonstrated commitment to modelling lifelong learning and continuous professional growth
- Demonstrated ability to be a team player and stay supportive and encouraging
- Some work on evenings and weekends may be required
- Use of proper body mechanics and successful completion of the Physical Readiness Test to ensure safe work practices are followed, including mobility, transfers, bending, lifting, pushing and pulling
- Valid driver’s license and use of a car on the job required
