Pay: $19.00 per Hour (Employer Estimate)
Job Type: Part-Time (28 Hours per Week)
Schedule: Monday to Friday, 8:30 AM – 2:00 PM
Work Location: In Person (Client’s Residence and Community Settings)

Key Responsibilities:

• Personalized Care: Assist with daily living skills, including medication administration, to support the client’s independence.
• Community Support: Accompany the client to appointments, activities, and job search efforts in Jersey City.
• Skill Development: Provide training aligned with the client’s Individual Habilitation Plan (IHP) goals, using creative and effective methods.
• Networking: Build a support network between the client and community members to foster inclusion.
• Documentation: Accurately complete required paperwork, such as claim form submissions.
• Planning Support: Participate in Circle(s) of Support for the client’s personal futures planning.
• Training Compliance: Complete and demonstrate proficiency in all required caregiver training programs.
• Crisis Management: Respond calmly and effectively to crisis situations with good judgment.

Qualifications:

• Education: High school diploma or equivalent.
• Age: Must be at least 18 years old.
• Driver’s License: Valid NJ driver’s license with a good driving record.
• Vehicle: Must have a reliable personal vehicle to transport the client.
• Background Checks: Pass drug testing, criminal background check, Central Registry, and CARI checks; subject to random drug testing.
• Training: Complete online and hands-on training, including fingerprinting.
• Compliance: Cooperate with PennReach, Inc. and Department staff during inspections or investigations.
